For service providers:   Making a referral is quick and easy.  Simply fill out the online referral form or the attached referral  form and fax to (651) 738-1881.  The referral will be processed within 1-2 days and the referral source will be informed of the outcome of the referral.  

 

Self-referrals:   You can call for a free phone consultation about whether or not our services are appropriate for you. At that time, an appointment can be set up.
