New Patient Forms:

New patients can save time during their first appointment by completing the Patient Information form prior to their visit. Simply print out the Patient Information form, fill in the information requested, and bring the completed form with you to your appointment.

Making A Referral

 

For service providers:   Making a referral is quick and easy.  Simply fill out the online referral form or the attached referral  form and fax to (651) 738-1881.  The referral will be processed within 1-2 days and the referral source will be informed of the outcome of the referral.  

 

Self-referrals:   You can call for a free phone consultation about whether or not our services are appropriate for you. At that time, an appointment can be set up.
